A three-level meta-analysis published in late 2025 went further, reporting a small but reliable effect of roughly 0.18 that was actually strongest under fed conditions during aerobic work, with a maltodextrin solution, a rinse of ten seconds or less, following a higher-carbohydrate meal (see [1]).

Read those numbers honestly. An effect size near 0.18 is small. A rinse is not going to turn a 22-minute 5K into a 20. It shows up most reliably in efforts of roughly 30 to 75 minutes at moderate to high intensity (see [2]), the range where you are working hard enough to want the lift but not long enough to genuinely need the calories a gel provides. For a hard parkrun, a 10K, a tempo where your stomach is unsettled, that is a real if modest edge. For a marathon it is beside the point, because there you have to swallow the carbohydrate, not just taste it.

So the practical picture is smaller and cleaner than the old story. You do not have to run fasted to get anything from a rinse, and the trials that fed their subjects a carbohydrate meal first still saw the effect. In those studies the solution was typically a 6 to 6.5 percent maltodextrin drink held for five to ten seconds. It is a tool for the short, sharp, uncomfortable efforts, not a substitute for the boringly easy aerobic base that should carry 80 percent of your weekly volume. Get the training right first. The mouth rinse is the last one percent, and now we know you can reach for it whether or not you have eaten.
