A new balanced-placebo trial in Nutrients this year finally separated the drug from the belief, and the result is worth pinning to your gym bag. Sixteen trained men lifted under four conditions: told caffeine and given caffeine, told caffeine but handed a dummy, and the two reverse combos, all at a 5 mg per kilogram dose (see [1]). The pharmacology did real work. Peak torque climbed about 4% at slow and isometric speeds, and total work rose roughly 4% across the board. But belief alone, thinking you had caffeine when you did not, produced no measurable strength gain. So the extra plate you feel on a good pre-workout day is mostly the compound, not the hype in your head.

Here is where I want you to reset expectations, though. That same trial showed muscle endurance, the reps you can grind out, did not improve significantly, a 3% bump that missed the mark and showed up in only half the lifters (see [1]). Zoom out to the broader literature and the strength effect stays modest but consistent. An earlier meta-analysis pegged caffeine's strength effect at a standardized 0.20, small but real, and slightly better for the upper body than the lower (see [2]). A 2025 review of movement velocity found the opposite pattern, larger effects on lower-body speed than upper (see [3]). Small effects, not a cheat code.

The wrinkle that fascinates me is what belief still did. Expecting caffeine, even on placebo days, raised blood lactate by about 0.7 mmol per liter on its own, roughly two-thirds of what the actual drug produced (see [1]). Your nervous system responds to anticipation, so the ritual of your pre-set routine has value even when the physiology is quiet. But lactate is not force, and the trial is clear that a hyped-up feeling did not translate into a heavier lift.

So treat caffeine as the small, honest tool it is. A 4% edge on your heavy sets is worth having on a max-effort day, and the study reached it with 5 mg per kilogram of bodyweight. Do not expect it to add reps to your back-off sets, and do not let a big scoop convince you the session was better than the numbers say. Log the loads, watch whether your top sets actually move week over week on something like the personal-records page, and let the bar, not the buzz, tell you if you are progressing.
