The weighted vest went from rucking accessory to bone-health protocol in about two years, on the theory that external load replaces the mechanical signal your skeleton loses as bodyweight drops. That theory has now been tested properly. The INVEST in Bone Health trial randomized 150 adults averaging 66 years old to twelve months of caloric restriction alone, restriction plus a weighted vest worn eight hours a day, or restriction plus supervised resistance training three times a week (see [1]). Vest load was titrated to replace up to 10 percent of the weight each person lost, and compliance was not the problem: the vest arm averaged 7.1 hours of daily wear and replaced roughly 78 percent of lost mass. All three groups dropped 9.0 to 11.2 percent of bodyweight. All three lost total hip trabecular volumetric bone density, between 1.2 and 1.9 percent, with no advantage for the vest and none for lifting either.

A null result on the primary outcome is usually where I stop reading. The secondary analysis is what makes this one worth the time. Posture had been measured directly with thigh-worn activPAL accelerometers, seven days at baseline, six months, and twelve months. Upright time turned out to moderate the effect, and the diet-only and vest arms differed significantly in how upright time related to hip areal density (p = 0.009) (see [2]). In the diet-only group, 30 extra minutes of daily upright time tracked with a 2.4 mg/cm2 decrease in total hip areal density. In the vest group that relationship flattened. Participants averaged about 5.3 hours upright per day at baseline, so most vest wear was sedentary wear, and a vest on a seated torso transmits almost nothing through the hip.

The vest did move one number. In a conference analysis of the same trial, resting energy expenditure at twelve months was up 146.9 kcal per day in the vest arm, while diet alone fell 52.5 kcal and diet plus lifting fell 89.7 kcal (see [3]). Metabolic adaptation is the standard toll of a long deficit, and the vest group was the only one that did not pay it.

The measurement lesson generalizes well past vests. Dose in this trial was defined as hours worn. The variable that actually carried the stimulus was hours loaded. Any protocol whose adherence metric is a proxy rather than the stimulus itself can read as 95 percent compliant while delivering close to nothing. If you run a vest experiment on yourself, standing and stepping time is the number to log alongside wear time, not instead of it.
